The book begins with a patient named Maria who teeters on
the brink of death from a massive head trauma. Her head was
swollen and she was in a deep coma. After a long rehabilitation
effort, Maria was discharged. The author documents a number
of heart wrenching stories of patients admitted literally on the
brink of death. The presentation is written from a medical
perspective with technical terminology interspersed throughout.

The book documents an American Academy of Neurology
meeting during May of 2001. One of the topics
presented was a study on the effect of music on
the brain. Research revealed that musicians had
more relative gray matter volume in both primary
sensory motor regions, the left basal ganglia, and
left posterior presylvian region, areas
that have been shown to be responsible
for organizing complex problems and for adaptation
to accommodate new data, as well as language skills
according to the book. In addition, the brainstems
of musicians are physically larger than non-musicians
and people with larger brainstems have better
communications skills. 1)

These findings merit further research with a view
toward developing new approaches toward the
teaching of communication, mathematics,
computer sciences and foreign language. A related
question involves whether or not physicians need
to know as much technical knowledge like the
intricacies of the Krebs cycle.

In an age where computer technology and artificial
intelligence is growing, the more technical
algorithmic aspects of medicine can be designed
into machine intelligence, Advice-Giving systems
and sophisticated inference engines. In my own experience,
a physician must do a number of important things to secure
the maximum benefit for the patient and save work later on.
These things include the following:

o empathy with the patient and his/her feelings and treatment of the whole patient holistically

o understanding of out-of-range test results by the patient

o completion of a comprehensive medical history which may include carryover from previous encounters

o follow-up visits and response to patient correspondence
o the careful use of second opinions when necessary

Overall, the book is a good recitation of real life
experiences in Emergency Medicine and other settings
where life and death decisions must be made quickly.
The presentation will appeal to a wide spectrum of
readers in the medical profession and in other
professions. The book is easy to read and very engaging.
